Breast Cancer Reconstruction can be performed to restore the breasts pre- or post- mastectomy. Depending on your body, needs, and preference, Dr. Theunissen offers many breast reconstruction options, some of which can be performed in conjunction with others to achieve desired results. Dr. Theunissen employs only the most advanced muscle-sparing procedures, which take skin and fat tissue, from your abdomen, buttocks, thigh area, and sometimes multiple areas. Muscle-Sparing procedures are considered the “gold-standard” now because women can recover faster, without sacrificing much needed, core-strength muscles as previously sacrificed  in older, dated procedures, such as with TRAM Flaps.

“Pre-” and Post-Mastectomy Breast Reconstructive Options

DIEP Autologous Flap Reconst | DIEP Breast Flap

DIEP Flap Reconstruction is performed at the same time as the mastectomy, which is sometimes referred to as pre-mastectomy, or may be performed at a much later date. It is likely the most widely known and performed muscle-sparing autologous flap method. It is performed by taking the autologous tissue flap from the lower abdomen and replacing the the breasts’ cancerous tissue with the abdomens healthy flap of skin, fat, and blood vessels from the abdomen. This is why it is often referred to as the “Tummy Tuck Flap” because of the added bonus of a Tummy Tuck.

PAP Autologous Flap Reconstruction

 

The PAP Breast Reconstructive method gets its name from the Profunda Artery Perforator. This reconstructive procedure restores the breast with a healthy autologous flap from the thigh, usually the back of the upper thigh. This helps to hide the scaring within the crease of the buttocks. The major added benefit women receive, aside from having new healthy breasts, is a thigh lift. The amount of upper thigh tissue available is a determining candidacy factor for the PAP Flap.

GAP Restorative Tissue Flap

 

GAP Flap Reconstruction uses an autologous flap of tissue from the buttocks to reconstruct and restore the breast or breasts. There are two types of GAP Flaps:

  1. sGAP (Superior Gluteal Artery Perforator)
  2. iGAP (Inferior Gluteal Artery Perforator)

The incision area and portion of the gluteal artery used for these two procedures is the main difference between the two. For the sGAP, the part of the gluteal artery used for this procedure is located more toward the top of the buttocks area and the incision will be closer to the top of the hips area.  For the iGAP, the part of the artery is located more toward the bottom of the buttocks area, and the incision area is discretely in the crease of the buttocks.

Hybrid Stacked Autologous Flap Reconstruction | Composite Stacked Flap

 

The Hybrid Stacked Flap, which is also referred to as Composite Flap Reconstruction, can include any combination of DIEP (abdomen), PAP (inner thigh) or GAP flaps surgeries to achieve excellent results by using natural body tissue. In order to reconstruct both breasts, composite tissue is taken from multiple donor sites. Combining tissue from the abdomen, inner or outer thigh allows Dr. Theunissen to give the patient a tummy tuck, thigh lift, and buttocks lift simultaneously while bringing the breast to restoration.

Tram Flap Reconstruction | Non-Muscle Sparing

 

Free TRAM Flap Reconstruction, which is not a muscle sparing breast restoration procedure, uses a flap of tissue containing blood veins, skin, fat, and muscle from the abdomen. TRAM stands for the blood vessels that are used for the flap – Transverse Rectus Abdominis Myocutaneous. Dr. Theunissen does not perform the TRAM flap because it uses muscle for the restoration of the breast, which is in many cases unnecessary. A breast derived from fat is going to feel more natural than a breast derived from muscle and fat. Tram Flap Reconstruction recovery time Vs DIEP Flap recovery time is also much in favor for the DIEP Flap and other muscle sparing procedures Dr. Taylor Theunissen performs.
